By my signature below, I acknowledge that I have voluntarily chosen to have my child participate in a Catoctin CrossFit class in which they will engage in strenuous physical activities on CCF premises. I understand that these physical activities involve certain risk and exposure to personal injury, which risk and exposure I voluntarily assume for myself and child.

I certify that my child is in good health, have no disability, impairment, injury, disease or ailment that would prevent them from using the CCF facilities, equipment and services and that I am able to use the same. I understand that my child is not obligated to perform nor participate in any activity that he/she does not wish to do, and that it is my right to cease their participation at any time. My child understands that if they feel lightheaded, faint, dizzy, nauseous, or experience pain or discomfort, they are to stop the activity and inform the CCF trainer. I give CCF and the CCF trainer permission to seek emergency medical services for my child if they become injured or ill with the understanding that I am responsible for any expenses incurred.

In consideration of the use of CCF’s facilities and service, the receipt and sufficiency of which is acknowledged, I hereby release in full and forever discharge CCF, its Directors, Officers, Managers, Owners, Members, Employees, Contractors and Agents, and all members and guests of CCF, whether acting officially or otherwise, on behalf of myself or any member of my family, our Representatives, Heirs, Executors, Administrators and Personal Representatives, from any and all liability, losses, damages (to person and/or property), claims, demands, and/or causes of action, (including costs and attorneys fees) whether foreseen or unforeseen, relating to or deriving from any injury to me, during or arising out of the use of CCF’s facilities or participation in any of CCF’s events (e.g., classes, seminars, etc). Parent/Guardian on behalf of Minor Child: By signing this Informed Consent/ Disclaimer of Liability/General Release and Waiver, I state that I am the parent/guardian of the CCF Guest named above and, having all necessary legal authority to do so, agree, understand and accept all terms, conditions, statements and incorporated documents contained and set forth in the above Informed Consent/ Disclaimer of Liability/General Release and Waiver on behalf of the CCF Guest who also executed this document.

I have read and understood the foregoing assumption of risk and release of liability and I understand that by completing the registration, it obligates me to indemnify the parties named for any liability for injury or death of any person and damage to property caused by my negligent or intentional act or omission. I understand that by signing this form I am waiving legal rights.
